  • Transparent and clear role for practitioners in service and patient quality and safety
  • Effective communication between practitioners and patients
  • Effective communication within the organisation which may include a timely reporting mechanism and no blame culture

Please use an organisation that you are familiar with or a sector as an example, to explore and discuss to what extent the above factors that are critical to improving the quality and safety of health services have been addressed and to what extent these efforts are effectively monitored and measured for continuous improvement.
